What to bring
- Any emergency department or urgent care records from the injury, including imaging reports
- A description of the mechanism — what happened, and what the first minutes afterward were like
- Your current symptom list, and what makes each symptom better or worse
- A list of current medications
- If the injury is connected to a claim, the claim or case reference
